
Our services in Leeds
The Viral Hepatitis Nurses team at Leeds Teaching Hospitals was established around 2002. At that time, the team consisted of two dedicated nurses working part time to provide a hospital based service working alongside Consultant Hepatologists delivering treat for Hepatitis C.
To meet the changing needs of the service and to engage with hard to reach patients in the local community outreach services were introduced.
The team operates outreach clinics in the Leeds, Kirklees (Dewsbury) and Wakefield areas and in–reach clinics to the four HMPs in West Yorkshire in addition to the hospital-based service.
The nurses are committed to providing the best practice to optimize treatment outcomes for patients with the support of the Consultant Hepatologists.
Referral to our service
Referrals to the service are received from the Leeds, Kirklees (Dewsbury) (Drug and Alcohol Services only) and Wakefield areas and the following sectors:
- Drug and Alcohol Services

- HMP Healthcare
- Sexual Health Services
The referrals to the Outreach Clinics is for Hepatitis C treatment only, the information below will assist you with getting your referral to the right department.
Drug and alcohol services
These referrals are for clients from Forward Leeds only and are for appointments in the outreach clinic at one of the Forward Leeds Hubs, Armley Park Court, Irford House, Seacroft or Kirkgate in Leeds city centre. Referrals can be made by completing the Referral Form – Forward Leeds and emailing it to the referral bookings team at [email protected].
Kirklees (Dewsbury)
These referrals are for clients from CGL Dewsbury (CHART) and are for appointments at the outreach clinic in Dewsbury. Referrals can be made by completing the Referral Form – CGL – Dewsbury and emailing it to the referral bookings team at [email protected].
Leeds Primary Care (GPs)

This is an Outreach Clinic where referrals are booked directly into the available clinic slots by the administration team at York Street Surgery for their clients only. In addition to the outreach clinic, a weekly drop in clinic takes place on Wednesday afternoons between 1pm and 3pm, no appointment required. This drop in clinic is only open to York Street GP Surgery or clients who attend York Street Forward Leeds services. HMP Healthcare
HMP Healthcare covers the four prisons in West Yorkshire (Leeds, New Hall, Wealstun & Wakefield Prisons), referrals are sent to the Leeds Teaching Hospitals Referral Booking Service. Wakefield Area Primary Care (GPs)
Referral from GP who would like their patients to be reviewed by a Doctor please use the Referral Form – Leeds Hospital and emailing it to [email protected]. If you would like your patient to be seen in an Outreach Clinic, referrals can be sent direct to the Viral Hepatitis Team. This can be actioned by completing the Referral Form – Wakefield, Castleford, South Kirby & Pontefract and emailing it to the referral bookings team at [email protected].
